Registration and history forms are provided when you schedule; completing them ahead keeps your visit focused on the conversation, not the clipboard.

Records & prescriptions
We can request prior records for you once you're scheduled. Prescription refills are handled through the portal or your pharmacy between visits.
Preparing for your first visit
Three things genuinely worth bringing: your current medication list (including supplements), any prior lab or imaging results, and brief notes on your cycle pattern and most disruptive symptoms. Everything else, we'll take from there.
